Starting Therapy
First Session:
In the first session you can anticipate that you will be asked to
supply information to the therapist about your life history and
your current life. Some therapists gather this information via a
form or by an interview. While this may seem frustrating, this information
that the therapist gathers will serve you later in therapy. Other
therapists gather this information over several sessions.
A consent for treatment/disclosure document should
be provided to you that explains to you several important factors
that are relevant to your therapy. This form should enclude these
topics: (Colorado is used as an example)
- The name; address; phone number of the therapist
- A listing of their degrees, credentials and licenses

- The address and phone number of the grievance
board
- A statement that sexual intimacy is never appropriate
and should be reported to the grievance board
- A statement that you may terminate therapy at
any time
- The limits of confidentiality. exlpore
